In an Atypical Mood Disorder one of the defining features is that symptoms from each of these clusters may be present at the same time, or alternate with each other, or at any one time only one symptom cluster may predominate. It is crucial to be aware of the whole diagnostic picture and not just respond to individual symptoms like problems with anxiety, sleep, depression, concentration, hyperactivity, or to just treat a cluster as if it were a separate diagnosis, like ADHD or simple Depression or Panic alone. The primary medications should be one of the mood stabilizers, with other classes if used alone must be done cautiously balancing short vs. long term risks vs. benefits. ADHD = only 15,16,17 alone & may respond to a stimulant like Adderal if no other significant symptoms.
Mood: Mood Stabilizers-Neuroleptic-Risperdal, Abilify, Zyprexa, Seroquel, Geodon, Haldol, Thorazine, Trilafon,- Anticonvulsants-Depakote, Tegretol, Lamictal, - Lithium Carbonate in a class by itself
1. [ ] Often unpleasant, irritable, nasty, mean, excitable, defiant to authorities, bullies peers
2. [ ] Short fuse, easily made angry, can’t calm self down in 20 min, rages lasting from minutes to hours
3. [ ] Quick and frequent mood swings, ups and downs of feeling states, Dr Jekle & Mr. Hyde
4. [ ] Too demanding & demands must be met immediately, difficult to live with, walking on eggshells
5. [ ] Can’t control anger, aggressive, violent, destructive to property, physically hurts others
6. [ ] Injures self on purpose, self mutilates, bangs head, bites self, punches walls
7. [ ] Brief periods of sadness, irritable and whinny, cries over small annoyances and hurts
8. [ ] Depression Equivalents-Low energy states increase with age, occur with physical illnesses like headaches, GI problems
9. [ ] Speech-Talks too much, too fast, difficult to follow, goes off on tangents , talks in circles
10. [ ] Sleep-Needs little sleep but not tired next , often unable to sleep because mind won’t shut down
11. [ ] Excessive involvement in pleasurable activities, that have high potential for painful consequences
12. [ ] Manic- Too happy, cheerful, enthusiastic, silly, giddy, goofy, overly friendly, even with strangers
13. [ ] Grandiose-Thinks too highly of self, over values abilities, sets unrealistic goals, bossy, needs to be in charge
14. [ ] Racing Thoughts-Subjective experience of too many emotionally charged thoughts, too fast
15. [ ] Impulsive-Not thinking of consequences of behavior, speech, goals, plans, and relationships
16. [ ] Distractible-Attention and concentration changes with mood, distracted by minor and unimportant things
17. [ ] Hyperactive-Mostly high energy states with increase of goal directed activities at home, work, school socially
Depression: SSRI/NRI/DRI’s-though helpful in short run may make mood & all symptoms remarkably worse depending on individual sensitivity primary brain chemistry- Prozac, Zoloft, Paxil, Effexor, Luvox, Celexa, Lexapro, Straterra Remeron, Trazadone, Welbutrin
18. [ ] Depression- sad mood, tearful, whiney, easily cries, irritable, feels empty and alone
19. [ ] Interest- diminished pleasure and interest in most things, don’t care, too bored attitude
20. [ ] Body Weight- significant weight loss or gaining, eating too much or too little, not dieting
21. [ ] Sleep- sleeping too much, too little, waking up cant fall back to sleep, never feels rested
22. [ ] Mind/Body Energy- as observed by others either too slowed down or speeded up
23. [ ] Fatigue- loss of energy, always tired, fatigued, no motivation, drive, ambition
24. [ ] Negativity- worthless, guilty, ashamed, negative about self, present , past and future
25. [ ] Concentration- diminished ability to think, remember, concentrate, mind goes blank
26. [ ] Hopelessness- hopeless , recurrent thoughts of death, dying, suicide ideas, attempt, plan
Anxiety- SSRI type, plus benzodiazepines may have paradoxical effects, lead to addiction- Ativan, Klonopin, Xanax, Valium, Tranxene
27. [ ] Social Anxiety -Fear , embarrassment of social, performance situation, with unfamiliar people scrutinizing
28. [ ] OCD-Worry of not doing rituals, hand washing, checking, ordering, counting, repeating, praying
29. [ ] Panic Disorder-Panic attacks, heart pounds, shaking, short of breath, dizzy, lightheaded, losing control fear
30. [ ] PTSD-Extreme fear, numbing, avoidance and re-experiencing trauma, abuse, neglect, catastrophes
31. [ ] Separation Anxiety-Worry over losing & leaving loved one, trouble being & sleeping alone, need some to go places
32. [ ] School/Work Phobic-Frequent school work lateness, avoidance, absences, due to illness, especially early in week
33. [ ] Psychosomatic- Always sick, headaches, migraines, stomach-bowel problems, easily fatigued, allergic, rashes
34. [ ] Generalized Anxiety-Too worried all the time , keyed up, on edge, tense, irritable, trouble falling and staying asleep
