In order to ensure a professional consistency of uniforms aboard the Dauntless, a program exists whereby crew-members may have their uniforms certified as "Correct and Accurate", referred to as a CAUC (Correct and Accurate Uniform Certification).
Obtaining a CAUC may count towards rank promotions, and is a requirement in order to apply for many Duty Stations aboard the ship. It is therefore in your best interests to ensure that any uniforms you own have corresponding CAUC's.
No. Wearing a uniform to an Away Mission does not count if it has not been certified. You may request certification at the mission itself, if one of the certifying Officers happens to be present.
A CAUC is issued after a successful inspection of the uniform, and it is found to meet the criteria set forward in the uniform regulations for that class of uniform. The criteria are listed under the descriptions for each uniform class in the pages linked below. Only uniform types included in the code will receive a CAUC.

Inspections may be conducted either in person or online.
Get into the uniform you wish to certify. You may certify multiple uniforms, one at a time.
Have someone take two photographs of you:
Full length from the front
Head-and-shoulders from the front, wide enough to show the combadge/SFI Name badge, if applicable.
Fill in the CAUC request form.
Should your uniform pass inspection, the CAUC will be issued to you electronically, and will be noted in your personnel file. If you fail inspection, you will be advised which details to correct on the uniform. Once the changes are made, you are welcome to reapply.

Yes. If you receive a rank promotion or you are reassigned to a different position, your CAUC will automatically expire, and you will need to apply to have each of your uniforms certified again.
Exceptions may exist where a Class C uniform may not need to change after a promotion or reassignment. When in doubt, ask your Department Head.
Yes. Some crew-members engage in simulated activities through online systems that allow avatars to wear Starfleet uniforms, such as Star Trek Online. For officially sanctioned virtual away missions that take place within those virtual environments, you will need to certify your avatar's uniform in order to qualify for the extra rank promotion points.
The procedure for certifying virtual uniforms is identical to that for real uniforms.
