Click on the sprocket icon in the upper right corner. Select “settings”. Scroll down to Signature. Create signature and confidentiality statement.

Log on to Data Dashboard to view the "Emergency List" for your school(s). These students have been identified as having life threatening health conditions. Contact the parents of these students for updates. Has anything changed? Does the condition continue to be life-threatening?
The Health Information Form is part of the On-line enrollment process for MMSD families. Parent/Guardians/Students see the Health Information Form on their computer screen. Returning families see conditions previously entered.
To Access Online Enrollment
Classlink --> MMSD created Apps --> Online Enrollment --> Select School from Dropdown Menu
Find emergency supplies and medications. Each health office should have the following:
At least one 0.3 mg EpiPen (check expiration dates)
At least one 0.15 mg Epi-Pen Jr. for elementary offices (check expiration dates)
At least one albuterol inhaler
Narcan kit
Stop the Bleed Kit
Emergency bag
Check in student medications, obtain & complete medication administration forms.

Your schedule will depend on your allocation, the number of schools assigned, and student need. Here is some guidance on developing your schedule.
Note that nursing assistants are required to work 5 days/week.
Locate the RED BINDER. The Red Binder is where individualized health care plans and emergency plans are filed. Make sure that the binder is stored in an easily visible space.
Create & update Emergency Health Plans & Individualized Health Care Plans as necessary. Notify teachers of students with significant or potential emergency health conditions, and train key staff in any applicable procedures (i.e. Epi-Pen administration).
Locate the BLUE BINDER. The Blue Binder is where sub plans, sub notes, student lists, school map and schedules are filed. Make sure that the binder is stored in an easily visible space.
Update sub plan, sub notes, student lists and schedules as necessary. Share a copy of sub plan with the health services administrative assistant.
