To find carbohydrate and allergy information:
Choose a school
Choose a meal (breakfast, lunch, snack)
The menu defaults to "Daily." For Detailed information about each menu item including serving size, menu components, allergens and carb counts, click on the teal food name.
For Weekly menus, in the "Select Categories" box, choose "Select All" and then toggle the "Show Nutrients" button to the "on" position. Click on the teal food name for detailed information.
On the Monthly menu, the information available is very limited. Monthly should not be used for calculating carbohydrates for meals that have multiple components (hot dog & potato wedges, hot honey chicken and rice, etc).
Food Services wants to be informed of a student's allergies even if they do not eat school meals.

Complete the Student Diet Modifications Form:
When you find out about a new student allergy even if the student does not eat school meals.
If a student is no longer allergic to a food and you have resolved the allergy in IC.
When a row is turned green, the dietician has entered the diet into PrimeroEdge and notified the building based Food & Nutrition Staff.

Before submitting this form to Food Services Department, please check the appropriate line in the upper right hand corner; never eats MMSD or participates in MMSD meals.
Complete the Student Diet Modifications Update Form to submit a completed Special Dietary Needs Form to the MMSD Food and Nutrition Department. The Special Dietary Needs form will be uploaded on the second page.
The Student Diet Modifications Update form will need to be submitted twice for students who eat school food. The form should be submitted as soon as a new student allergy is reported to the health office. It will be submitted again when a completed Special Dietary Needs form is received from the doctors office.
