For an appointment, if the student has BadgerCare, health offices should use VEYO (see more information below). For students with PAK, call the clinic to inquire about transportation. If the appt is urgent then you can use a voucher.
If a child is sick, and they use special transportation (in a 504 or an IEP), the health office should be connecting with Laura (ext #22899) in transportation to see if they can pick up.
A reminder that our cab funding is used to get people to appts. VEYO will not take families to the vaccine clinics because they are at schools, not clinics.

Primary Access For Kids Program
Primary Access For Kids Program
Primary Access For Kids is an initiative of the Dane County Health Council. Although this is not health insurance, this program provides primary care and care for illness and injury to children who are not eligible for health insurance. Care is provided at no charge. It does not cover hospital care. Each MMSD school is connected to a specific health clinic. When submitting a PAK referral to clinics, please include immunization information.
PAK Information for nurses and families
In this case of unaccompanied minors, UW Health needs a Verbal Communication form filled out to legally share health information with the person sponsoring, or responsible for the student.
Process for Referring Students to Primary Access for Kids Program Make sure to send all PAK forms to Nancy Koch Meyer for tracking purposes.

Foundation for Madison Public Schools
In March 2004, Group Health Cooperative donated money to the Foundation for Madison’s Public Schools to establish an endowment fund in Dr. Russell Hess’s memory. Because of this donation, we have $2000 available for emergency health needs each school year.
School Nurses must request approval to use these funds prior to purchasing. Please reach out to the Health Services Admin Assistant for approval.
