For urgent dental needs (pain, swelling, drainage) please fill out the Healthcare Assistance Request form and then call the Family Health Liaisons who will work with Access Dental to secure an appointment for the student.
Please make sure to find a small space for the Celebrate Smiles team for registration.
Enter with a "Yes" or "No" in the spreadsheet ONLY if you get a paper consent form.
Make sure the consent form is complete: Health history and signed by the parent or legal guardian.
Let your principal, teachers, and custodian know about Celebrate Smiles so they know the program will be in the building.
Ask your social worker and BRS to help you promote Celebrate Smiles. They have connections to your families.
Please ask your school secretary to post the flier to your school's Facebook.

Celebrate Smiles website, available in English or Spanish, can be a resource for parents who would like more information. Consent forms are available on this site if a parent needs to print one out and send it back to school, or if schools need a few extra. Printed forms will always be provided to schools through ACHC.
