What should families bring to the clinic:
Completed and signed CONSENT form (English), (Spanish)
Completed screening questionnaire (English), (Spanish)
MMSD Immunization Record + Any additional immunization records
Their child’s insurance card, if they have insurance. There is NO OUT-OF -POCKET COSTS!
However, SSM Health bills other insurances for the cost of each vaccine dose administered to their member.

Infinite Campus and WIR are able to exchange immunization dates. Parent permission is required for that exchange to happen, so for some students, you will need to check WIR for immunization dates and enter them manually into Infinite Campus. You will also need to communicate with parents of students who have moved from out of state to obtain their immunization dates/immunization records.
WIR User Agreement must be reviewed and signed prior to gaining access to the Wisconsin Immunization Registry. Signed agreement should be sent to the Health Services Administrative Assistant.

Steps 1, 4, & 5 need to be completed for a waiver. If the student has a medical waiver, they need an MD signature. Parents must indicate which vaccines they are waiving. A parent cannot waive a vaccine that is not currently required which means parents of elementary students cannot waive a Tdap. They will need to fill out another waiver upon entering 7th grade if they wish to waive Tdap.)

Sometimes records can be obtained through state registries. CDC link lists states and contact information.
For Illinois immunizations, please complete this Immunization Record Request Form if you would like to obtain records or the parent could go to https://idphportal.illinois.gov/s/login/SelfRegister to access records themselves.
If referring a student to PHMDC, fax or email their entire immunization record to Erica Rabehl, Public Health Aide, at (608) 261-9606 or ERabehl@publichealthmdc.com and include the date of their appointment.
If a student is a refugee, fax their entire immunization record to Erica even if they are not currently due for vaccine and make note that they do not currently have an appointment. This will allow PHMDC time to translate the remainder of the record (if needed) and enter the record in WIR so any clinic seeing this student will have the most up to date information for all vaccines.
Katy McMahon at GHC works only with refugee families and can assist with determining vaccine needs, set up a clinic appointments for the entire family, and arrange transportation, if needed to attend the appointment.
