We are enhancing our partnership with ConnectRx Wisconsin. We will meet monthly with their staff of community health workers to discuss any of our pregnant/parenting students that their agency is working with. If you have a Black pregnant or parenting student that hasn't been referred to this program and they have needs regarding any social determinants of health, you can assist the student by communicating with their healthcare provider and requesting a referral to the Connect Rx Wisconsin program. If you have difficulty facilitating this referral through a healthcare provider you can forward the name to Stephanie Gramann and I will inform the program regarding the need for a referral.
