MMSD elementary nurses have the opportunity to improve the care coordination of students with acute and chronic health conditions. With parent/guardian authorization, nurses have access to UW Health, Access Community Health Center, Group Health Cooperative, and SSM Health electronic medical records and the ability to communicate electronically with healthcare providers through an "In Basket" messaging system for asthma concerns or verbally by phone for any other health conditions. The UW program, called School Health Care Link Partnership, and the GHC project, called GHC SchoolConnect, encompasses any health condition that may impact a student during the school day.

Requesting MD Orders:
GHC-SCW and SSM Health: can request orders for any health condition electronically.
UW Health / Access: Requests for orders related to asthma management can be made via the "In-Basket" messaging; however, medications and treatment orders for other health conditions should follow our usual guidelines.
Printing: Individual and emergency health plans and MD orders may be printed for all respective health systems; however, provider notes are not to be printed or shared with other school staff or parents/guardians.
Parent/Guardian Record Requests: Do not review or interpret student electronic medical records for parents/guardians. Parent must contact their child's health care provider to obtain information found in Epic.
Passwords: need to be maintained in order to remain an active user. Inactive users will lose access and need to re-enroll, if access needed in the future.
HIPAA Training: is required annually for all users of the EMRs, available through the Talent Portal. You will be reminded annually when you are due.
After confirming a student's enrollment in Epic, check the EMR indicator box located in Infinite Campus.

Upload the consent form through the UW Health In-Basket function. New Message - use the drop down arrow to select: New CRM --> Topic: CL New UW- MMSD Attachment. Add the consent as an attachment. The consent will need to be in a PDF format to upload.
Both user agreements are signed electronically during the initial login process.

Upload the consent form through the GHC In-Basket function. New Message --> Customer Service --> Topic: GHC EpicLink - School Connect Patient Request. Fill out the required boxes, you can put 0000 in for the Social security number. Add the consent as an attachment.

For SSM Health Only: Legally, you may not access a chart until you have a signed consent and the consent is uploaded into Infinite Campus under the respective student's Health Documents. You do NOT upload or send the SSM Consent anywhere.
Users need to attest that that will abide by the confidentiality statements and user agreements while accessing, viewing, and printing documents from EpicCare Link. The confidentiality statement and user agreement are provided electronically when users login to EpicCare Link for the first time.
