UW provided Education Resources:
School Nurses play a vital role in educating students about Diabetes and its management and supporting students as they transition from a child/family-centered model of health care to an adult/patient centered model of health care. Health care transition involves planning, transfer and integration into adult-centered health care. MMSD Health Services is partnered with the UW Pediatric Diabetes team to provide high quality education resources and a program that supports all of our students in learning about diabetes.
At the elementary level, students learn about their diabetes and management.
At the middle and high school levels, students work with their schools nurses to build their independence in diabetes care and their ability to navigate complex health systems so they are prepared to transition into adult-centered health care.
Middle and High school students fill out a survey to self assess their Diabetes knowledge and skills. Students then work with their school nurse to identify gaps in knowledge and chose a topic to focus their learning on during the school year. School nurses then meet with students throughout the school year to provide education using the UW Diabetes Resources for School Nurses.
School Nurse spends time with student watching at least 1 but up to 4 Diabetes Besties Videos prior to Spring Break.
