Supporting Preceptors

Each term we send each preceptor a Welcome packet that includes a letter from the Program Director & Clinical Faculty Director, a Quick Start Guide, and Preceptor Handbook. In the letter, we invite our preceptors to become Sponsored University Affiliates (SUAs).

If your student's preceptor wishes to obtain SUA status and gain access to these resources, please download and email them the following forms with a cc: to our Program Coordinator, Emma Benson-Xu, who will facilitate the process: Please note the preceptor will need to submit a copy of their photo id as well.

For preceptors who would like to request CE for their time precepting, you can cut/paste this link: pending.  Send requests directly to CFD Katie Daily in interim.

*This form is sent out to preceptors at the end of each clinical term. If they are requesting for the current term, advise them to wait until the end of the term when all hours have been completed.
