When evaluating clinical SOAP notes assigned for NURO 6511, 7672, and 7684, CFAs should take the following steps:
Review the SOAP note and annotate feedback directly in GU's Canvas.
View the SOAP Note Rubric to determine scoring for the assignment. This can be found in the NURO 6511 course in Canvas.
For courses Fall 2022 and beyond: Enter the scores directly into Canvas under "View Rubric" and click save. The score will automatically trigger a complete or incomplete grade. If the student scores below 80% please note the due date for resubmission in the comments.
Provide an opportunity for students to discuss feedback or debrief the clinical encounter at your next standing meeting.
The goal of the rubric is to provide students with clear, consistent feedback on specific clinical documentation competencies. A secondary goal is to have objective thresholds for when a revised or new note is required.
Save a copy of the SOAP Note Rubric that will be available to students in the course syllabus. Please familiarize yourself with the criteria for each component.

Timely review of SOAP notes is important! Please coordinate with your student(s) regarding SOAP note submissions and returned feedback. Grade and post all SOAP notes within 1 week.
If a student has submitted 2 notes without significant improvement, please loop in the Course Coordinator for additional support.
