How to add a form to a popup on a website

Go to Channels > Focus Items

Click +New to add a focus item

Enter name of the focus item and URL where you want it to show

Optionally, select Category

Click Builder

In the Builder page, select "Collect data"

Click "Yes" to animate

Select when you want the popup to appear. For example, click "After scrolling to middle" if you want the popup to show when the visitor reaches the middle of the page.

Enter how long before the popup appears. For example,  if you enter 10 sec,  when the visitor reaches the middle of the page, the popup will appear 10 secs after that.

Select how often do you want the popup to appear. For example, click "Once per session" so the popup will only appear once per visitor's session on the website.

Click "Yes" to stop the popup when the lead has filled out the form

Select popup style. For example, click Modal style and Middle placement if you want the popup to float in the middle of the website where it will get the most attention.

Click Content and select the form that you want to place in the popup.

Optionally, you can change the colors, font and add HTML and CSS to customize the popup.

Once you're done, close the builder.

Click Publish and save the focus item.
