Service Plan is created with an educational team. Student has to qualify for special education. The plan should be reviewed annually.
If the student does not have a current evaluation that is valid for the team to use to determine appropriate services, then the school will have to provide an evaluation.
Only if a student would qualify if they were in the regular school setting, does the student qualify for special education services.
Plan Basics:
Eligibility - must meet special education eligibility as if the student were in regular school setting.
Needs - what educational needs does the student have
Services - what educational/special education services are appropriate for the student; must correlate with the student's needs
Services do not have to be at the student's home
Services can be at school, the district may have to provide transportation
Duration of Services - when would the services start and end (remember funding is based on the current year's proportionate share in the IDEA grant)
Goals/Activities - what will the student be working on during these services (this is not an IEP and doe not need to be annually measured goals or have baseline data)
