Date of Eligibility Determination: Date of Final MET to determine Eligibility
eIEPPRO has a generic eligibility form which gives all the choices without having to pre-determine outcomes before the meeting.
Also, specific eligibility forms help to so how the student actually qualifies - more documentation to eligibility.
A1. Eligibility Determination: Select appropriate responses
Two questions have to be answered to receive special education:
Does the student have an educational disability as defined by federal/state regulations - do they meet the criteria?
Does the student NEED specialized instruction to progress in the general education curriculum?
If the answer is no to either question the student does not qualify.
Options if student does not qualify:
Identify supports in the general education curriculum
Does the student qualify for a 504 plan?
A2. The team determined the following: Select final outcome
B. Areas of Eligibility Select eligibility based on data and team determination
Follow eligibility guidelines in Educational Disability
Only need ONE primary eligibility
Note: Written medical verification is required for the following eligibilities: OHI, OI, TBI, VI and HI.
This should be part of the Assessment process
C. Multiple Disabilities:
Multiple Disabilities Worksheet: complete for students that qualify for MD or MDSSI
Clarification: Optional
Multidisciplinary Eligibility Team: complete at time of decision for eligibility
only students that qualify for SLD do the team members have to give documentation if they AGREE or DISAGREE with the team decision (consensus)
If a team member DISAGREES then written documentation has to provided to the school district as to why the team member does DISAGREE
Parents do NOT have Veto power - it is a TEAM decision
