When the Considerations (E4) Form is completed and the types of assessments are checked, the Permission to Evaluate form can be printed for parents to sign to give permission to evaluate.

eIEPPRO has a generic Permission to Evaluate Form that is good to use so that the team is not making a pre-determinate of which tests are to requested by completing form E4 prior to the meeting to have parents sign

As as the eIEPPRO generic form, the District form can be used and has the additional information for better parent permission according to the state
