Side lying on strong side
Description: a compensatory swallowing strategy that will reduce pressure on residue left after a swallow, allowing it to localize and gather until purposeful swallow and reduce aspiration risk.
Impairment targeted: Patients with pharyngeal residue due to reduced pharyngeal contraction may benefit from side lying posture while swallowing for nutrition/hydration.
Swallowing Stage: Pharyngeal
Example of Goal: In two weeks, Susan will self feed a soft & bite-sized meal without overt signs/symptoms of aspiration in 80% of opportunities given frequent moderate verbal cues to utilize swallowing strategies.
Outcome: Less aspiration for patients experiencing dysphagia
Good for patients:
Who are unable to sit upright (90 degrees).
Activity breakdown:
During a meal, the patient is instructed:
Lay on side throughout meal on semi-reclined or fully-reclined bed/chair
Hierarchy cueing system: A patient may require (min/moderate/max) visual, verbal, modeling, and physical cues to maintain appropriate bolus size and bolus flow during a meal.
Evidence-Based Practice: Side lying has been proven to be an effective compensatory swallowing strategy for patients with dysphagia challenges (Groher & Crary, 2021).
