Evidence-based Practice: "The training programs LACE, BrainFitness, and clEAR are training programs with various auditory tasks that also include cognitive or audiovisual tasks...such varied paradigms seem to be the most efficient training programs to improve speech perception (in noise) with additional benefits for auditory cognitive capacities...this supports the conclusion of a recent meta-analysis and review looking a the effect of auditory or cognitive training to improve cogntive skills in elderly individuals with hearing impairment (Lawrence et al., 2018)...However, some caution is necessary, given that some studies using a similarly intense training protocol did not observe any beneficial effects of training on speech perception or cognition (Barcroft et al., 2016; Saunders et al., 2016)" (Stropahl et al., 2020).
clEAR Auditory Brain Training is composed of games targeting the following: phoneme discrimination, recognizing frequently used words, auditory attention, auditory processing speed, bound morpheme identification, discourse comprehension and auditory memory. (clEAR, n.d.)
The Treatment below is for Bound Morpheme Identification
It is challenging for individuals who are hard of hearing to grasp the ending of a bound morpheme (e.g., can vs can't) during a spontaneous fast speech conversation; which can impact the overall comprehension of the whole conversation (Stropahl et al., 2020).
Goal: In 6 months, while using the clEAR Bound Morpheme Identification game, the patient will independently identify the appropriate bound morpheme (e.g., can vs can't) when an audio recording is heard in all levels (i.e., word, phrase, sentence) with 80% accuracy across 2 consecutive sessions as measured by clEAR and SLP data collection.
Impairment: Adults with hearing loss
Activity Breakdown
Step 1: The patient will be presented a word or sentence.
Step 2: The patient will select the bound morpheme that was heard. Repetitions may be used.
Step 3: The complexity increases (i.e., longer sentences) as the patient begins to select the right answer.
Hierarchy Cueing
Independently: The patient identifies the correct bound morpheme at the word/sentence level.
Repetition: The patient may repeat the word/sentence.
Providing Answer: The LACE program provides the correct bound morpheme on the screen.
