Evidence-based Practice: "The training programs LACE, BrainFitness, and clEAR are training programs with various auditory tasks that also include cognitive or audiovisual tasks...such varied paradigms seem to be the most efficient training programs to improve speech perception (in noise) with additional benefits for auditory cognitive capacities...this supports the conclusion of a recent meta-analysis and review looking a the effect of auditory or cognitive training to improve cogntive skills in elderly individuals with hearing impairment (Lawrence et al., 2018)...However, some caution is necessary, given that some studies using a similarly intense training protocol did not observe any beneficial effects of training on speech perception or cognition (Barcroft et al., 2016; Saunders et al., 2016)" (Stropahl et al., 2020).
There are 5 components within LACE Auditory Training, which are: speech-in-noise training, rapid speech training, competing speaker training, auditory working memory training, and communication strategies. (LACE, n.d.)
The Treatment below is for Rapid Speech Perception Training
Understanding rapid speech is challenging for individuals with hearing loss and rapid speech perception training uses a synthetic voice to practice understanding fast speech at the sentence level; the length or speed of the sentence increases as the individual improves (LACE, n.d.)
Goal: In 6 months, when presented with a synthesized voice recorded sentence, the patient will independently verbally repeat the sentence aloud correctly in 8/10 opportunities across 2 consecutive session as measured by LACE auditory program and SLP data collection.
Impairment: Adults with hearing loss
Activity Breakdown
Step 1: The sentence will be spoken by a synthesized voice recording and the patient will be asked to repeat as much of the sentence aloud.
Step 2: The sentence would self-monitored and respond to yes/no when asked if he/she fully understood the sentence presented.
Step 3: As the patient improves, the sentences and/or speed increases.
Hierarchy Cueing
Independently: The patient imitates the same sentence without any errors.
Repetition: The patient may repeat the sentence
Presented with sentence in writing: The LACE program provides the sentence on the screen and it would ask if he/she fully understood the sentence.
LACE: Auditory training that combines 4 tasks to help strengthen listening and comprehension skills with speech in noisy environments, being able to understand rapid speech talkers, strategies to help focus on the person you want to listen to, and improving auditory memory to be able to fill in the blanks with target words (LACE, n.d).
