Self-monitoring is imperative to an aural rehab plan for a patient with hearing loss. Daily logs can be used with patients to help them assess their communication interactions (Tye-Murray, 2008).
Goal: In 6 months the patient will use the Self-Assessment of Communication (SAC) to identify and monitor antecedents to communication breakdowns once daily in 4/5 weekly interactions with 80% accuracy as measured by self, communication partner, and/or SLP.
EBP: Convery, et. al, (2019) was able to indicate the participants of the study reported, "greater satisfaction with emotional-well being, social participation, sound quality and performance of their hearing aids" (Convery, et. al, 2019).

Step 1: Clinician will discuss the importance of self-assessment for the patient with hearing loss
Step 2: Patient and communication partner will self-log daily

Independently: The patient demonstrates understanding to independently fill out daily logs
Gestural Cue: The communication partner provides a gestural cue that indicates reminder to fill out log
Verbal Cue: The communication partner provides a verbal cue to fill out log
Model: The clinician/communication partner will model how to use/fill out SAC
