For images/study corrected or moved or images under wrong patients, please submit a ticket to Service Desk using the template below.
Any other records updates such as patient address, mobile number, home number, email address, spelling of name made in error or spelling of name to match insurance card, can be handled by scheduler not IT Dept.

Please include the following information in the BODY OF EMAIL:
Ø MRN(s) impacted
Ø Full Name (for each patient impacted, if more than 1)
Ø Patient date of birth
Ø Accession(s) #
Ø Date of Service
Ø Details – What happened, why did it happen (if known)
Ø Other comments/information
This will route your request automatically to the correct team to begin getting the correction started.
