Record Merging
When submitting a request for a patient record to be MERGED (Same patient with multiple MRN records), please submit a ticket to Service Desk via email by listing the following:
EMAIL TO: Servicedesk@simonmed.com and himteam@simonmed.com
With the SUBJECT LINE: “ATTN HIM – MRN MERGE DUPLICATE RECORDS”
Please include the following information in the BODY OF EMAIL:
First MRN:
Patient First and Last Name (As it appears in EXA):
Patient DOB (As it appears in EXA):
Second MRN:
Patient First and Last Name (As it appears in EXA):
Patient DOB (As it appears in EXA):
MRN to keep:
MRN to discard:
Once you receive a ticket number back from Service Desk (via email) it is important that this information is documented in the accession under Scheduling Notes.
This will route your request automatically to the correct team to begin getting the correction started.
