MEDICAL RECORDS FORMS (When patients want to obtain records from SMI)

We have updated our “Authorization to Obtain” forms. Please visit the portal (https://portal.simonmed.com/), and go to the Medical Records tab (see screenshot below) for updated forms.  We have two versions that are linked in the portal, there will be one for the Eastern states and one for West and Central states.  Please be sure that you are filling out the correct form to ensure that it is received and processed in a timely manner.  Please replace any printed/physical forms at hand with old information, with the new forms.
