Adding a Referring Physician

When you cannot find the patient's referring physician in EXA, you will need to submit a request to have the physician added.  Please submit a ticket to Service Desk via email by listing the following. 

EMAIL TO: Servicedesk@simonmed.com

In the Subject Line: ATTN RIS: Provider (Add/Update/Retire)

Please include the following information in the BODY OF THE EMAIL:

 

Referring physician (First and Last):

NPI:

Practice Full Address:

Office Phone Number:

Office Fax Number: (This is required unless an EMR connection is in place)

Accession Number:


You can find the referring physician's NPI (National Provider Identifier) by utilizing the NPI Number Lookup link on the SimonMed portal under the Contact Center section.  See first screenshot below. 


Once you receive a ticket number back from Service Desk (via email) it is important that this information is documented in the accession under Scheduling Notes.  See second screenshot below.
