Fill-up the PhilHealth Member Registration Form (PMRF), tick FOR UPDATING on the upper right-hand corner of the PMRF. You can select either Professional Practitioner or Self-Earning Individual / Individual under Member Type.
Submit properly filled out PMRF to the nearest PhilHealth Office.
They will charge you the current minimum (for 2025) of PhP 500. Pay for the first one at the office; you can also pay for the rest until December.
You can also pay online through the PhilHealth Portal.
***Tip: pay multiple months if possible to minimize forgetting to pay
Fill-up the Member's Data Form (MDF)
Visit your local HDMF office and submit the filled MDF.
Starting February 2024, the voluntary contribution is PhP 400/mo. You can pay the first one at the office or through Virtual Pag-IBIG

Pay online as Voluntary Member; No need to visit the local office for information update.

***Tip: This is the most expensive monthly gov't fee, but as a voluntary member you can just choose to pay from the list of contributions what you can afford.
Requirements to register your Freelance Business:
Taxpayer Identification Number (If you don't have a TIN yet, BIR will issue your TIN during registration.)
BIR Form 1901 - Application for Registration - 2 Copies
Part VII of the BIR Form 1901 - Receipts and Invoices. Ask for help from the BIR Officer on this part. Ask the BIR officer processing your registration if you want them to process the Service Invoice printing.
BIR Form 1905 - Application for Registration Information Update/Correction/Cancellation - 2 copies
Transfer of RDO if your old RDO is different from your new RDO
Valid ID - with birthdate and address
Occupational Tax Receipt (selected RDOs only)
Get this from your Municipal Hall
Service Invoice: 10 Booklet
The minimum cost of 10 booklets with 50 pieces of receipts per booklet is around 1,500 to 2,000.
