Fill-up the PhilHealth Member Registration Form (PMRF), tick FOR UPDATING on the upper right-hand corner of the PMRF. You can select either Professional Practitioner or Self-Earning Individual / Individual under Member Type.
Submit properly filled out PMRF to the nearest PhilHealth Office.
They will charge you the current minimum (for 2025) of PhP 500. Pay for the first one at the office; you can also pay for the rest until December.
You can also pay online through the PhilHealth Portal.
***Tip: pay multiple months if possible to minimize forgetting to pay
Fill-up the Member's Data Form (MDF)
Visit your local HDMF office and submit the filled MDF.
Starting February 2024, the voluntary contribution is PhP 400/mo. You can pay the first one at the office or through Virtual Pag-IBIG

Pay online as Voluntary Member; No need to visit the local office for information update.
You can change the amount every month. https://member.sss.gov.ph/members/
***Tip: This is the most expensive monthly gov't fee, but as a voluntary member you can just choose to pay from the list of contributions what you can afford.
Requirements to register your Freelance Business:
Taxpayer Identification Number (If you don't have a TIN yet, BIR will issue your TIN during the registration.)
BIR Form 1901 - Application for Registration (2 Copies)
Valid ID (with birthdate and address)
Occupational Tax Receipt (Selected RDOs only)

Official Receipt: 10 Booklet: Php 2,050.00
Register in the RDO assigned to your ‘business address'. For most freelancers, their business address is their home address.
