Important: The alcohol permit needs to be filed at least one week before the event!

What you will need:

• A faculty or staff sponsor who agrees to be at the event and prevent underage young'uns from drinking booze.

• Signature of the department chair (Lynn Huntsinger)

• Signature of the facilities head

What to do:

1. Reserve your location.

2. Fill out the form (attached below)

3. Get the appropriate signatures and make sure your faculty sponsor is all set

4. Email the form to ucpdspecialevents@berkeley.edu at least one week in advance of the event.

5. List your event in the department calendar (if appropriate)

6. Prep your signup sheets (attached below) and try to get everyone at your event to sign them.

7. When you receive the form from UCPD, forward a copy to facilities. If the grad student coordinator is sponsoring the event, also send a copy to the grad student coordinator.

8. After the event, scan your sign-in sheets.
