Unless you are a new hire, the last day of your coverage will be April 30, 2021 and the first day of your coverage will be May 1, 2021. For newly hired employees, insurance coverages begins the first month after your start date.

Review the benefits guide. This has a lot of pertinent information that you can view related to your insurance coverage.
If you have dependents, have their SSN and DOB on hand if you’re adding them for the first time.
For your Life Insurance policy, naming your Primary and Secondary (Contingent) beneficiary is super important!
o Note: You don’t need their SSN to name that individual, just their DOB. Although, it’s good to have.
Selecting over $80k in Voluntary Life coverage? This will require filling out an Evidence of Insurability (EOI) form from People Ops. To get the process going, email us at people.ops@levelset.com or directly Slack Terah and/or Madeline for the form
After Open Enrollment you’ll be able to access your digital card through Aetna.com which will be available to you approximately the last week of April/ first week of May (2021). Your physical insurance card would be sent approximately 2-3 weeks (mid-May)
