Medical
Levelset pays 100% of your medical insurance premium for a high-deductible HSA plan, beginning the month after your start date. Levelset also offers two co-pay plan options starting at ~$15 per paycheck.
You may add partners, spouses, or dependents at your own cost.
Your coverage begins the first of the month after your start date.
Your coverage ends on the last day of the month when you leave.
COBRA coverage is available to most employees when they leave (whether they resign or are terminated).

Levelset does not pay for dental or vision insurance, but makes a group plan available to you at an affordable rate.

Each employee can choose a fitness related incentive worth up to $100 per quarter (which may be a gym membership, fitness tracker, etc.) as an incentive and decide a goal which is approved by the fitness committee. If the employee does not meet the goal, they have to repay the amount of the incentive. The top performer each quarter receives an additional prize worth up to $250.

With the Get Caught Reading program, LevelSet will pay for the fiction or non-fiction book that you read. All you have to provide the title of the book you read, a short blurb about what you learned, and if you'd recommend it! That's it!
LevelSet is sponsoring employee sponsored programs available at no cost to you and all members of your household. That includes dependent children up to age 26, whether or not they live at home. Aetna will be able to help address grief or loss, personal or workplace concerns, legal assistance and financial coaching.
