Consent:

• Discuss the risks & benefits of the procedure.

• A provider who knows and performs the procedure should consent

• A provider who obtains consent should document the consent completion in the H&P note

• If two independent procedures are performed simultaneously, two separate consent forms should be obtained.

Witness:

Any clinical teammate may witness the consent form by asking the following questions: 

• Did a healthcare provider explain the operation or procedure to you?

• Did a healthcare provider explain that selected tasks may be performed by an assistant(s)/resident(s)?

• Did a healthcare provider explain alternative procedures and treatments and their risks and benefits?

• Have you given your consent for the operation or procedure(s)?

• Have all of your questions about the procedures(s) been answered?
