• For patients with Lymphoma, Myeloma, Leukemia, MDS, and MPD who undergo port or all other tunneled central venous catheter (CVC) placement.

• Tunneled Hemodialysis Catheter

• Tunneled CVC placement in a high-risk patient.

• Tunneled CVC exchange

• For patients with current indwelling tunneled CVC who are getting an additional CVC placement.

• Ancef - 1st choice 

• Clindamycin - 2nd choice

• Vancomycin - 3rd choice

Dosages

• Ancef

  • 1 g IV: body weight < 80 kg

  • 2 g IV: bodyweight 80-120 kg

  • 3 g IV: body weight > 120 kg

• Clindamycin 900 mg IV

• Vancomycin 15mg/kg IV (max 2g)
