Learn more about Training for the Care Navigation Program

Now accepting applications for the Winter 2023A Rotation!

  1. Complete your application no later than January 21st.

  • If you are a Health Scholar, you can find the application on CORE ELMS.

  • If you are not a Health Scholar, you can apply here.

  1. Schedule an interview with the Care Navigator Team to take place between January 23rd-31st.

  • We are looking for personable, confident and articulate applicants who will create positive experiences with patients and the Case Management Team.

  • Interviews will take place via Zoom.

  1. Attend 2 training dates and pass the Care Navigator entrance exam.

  • Training Day 1: February 5th (time TBD) via Zoom

      • Will consist of:

        • Introduction to Care Navigation and Population Health basics

        • Learning how to navigate CORE ELMS & sign up for shifts

  • Training Day 2: February 12th (time TBD) on site at St. Joseph Hospital

      • Will consist of:

          • Care Navigator Exam

          • Brief EMR (electronic medical record) training, hospital tour and DST (department specific training)

  1. Attend 2 four-hour shadow shifts between February 13th - 28th.

  2. Begin shifting in March 2023!

If you are not currently a Health Scholar, please refer to the COPE website for information about clearance and tuition.

What is the Care Navigator Commitment?

We currently offer 2 program duration options:

  • 6-month commitment of 2 shifts per week (for a total of 8 shifts per month)

  • 9-month commitment of 1-2 shifts per week (for a total of 6 shifts per month)

Note: if you are a Health Scholar, you may shift concurrently on Care Navigator and a Health Scholar unit during your 2nd rotation as a Care Navigator.

On-Site Care Navigator Training at SJO

    • Please park above the 3rd floor in the Cancer Center Parking Structure.

    • Parking is free after 5pm and on weekends. COPE events are scheduled with this in mind - please park at the correct structure!
