Flu Season is here and everyone requires some type of documentation!
Deadline for Influenza Documentation
OCTOBER 31ST, 2022
You do not need to physically submit your flu documentation. Please follow the protocols listed below.
You have 2 options for Flu Documentation:
1) Complete the flu attestation after receiving the vaccine at a clinic/your primary care physician’s office
2) Complete the flu declination and wear a mask at all times in the hospital
Info for Vaccination
Receive flu vaccination and proof of vaccine administration
Upload copy of proof of vaccine administration to CORE under “Flu Seasonal Vaccination”. (Use the replace button on the right if available)
Wait for Health Scholar Administration Team to contact you regarding flu sticker pickup
Note: Please ensure that your NAME and DATE OF BIRTH are visible on all documents that you upload to ensure that all forms can be received and verified by our team.
