Welcome to the Administration Request page!

Here you can find information about various requests you would need our Admin branch's assistance on!

All the forms outlined below will require you to log into your CORE ELMS account and access the Electronic Forms section on the bottom left of the page. Once there you will simply need to open the form of your choice.

Hours Audit

Want to know your total hours completed thus far? Well it is time to submit your Hours Audit Request!

The Hours Audit is a formal letter detailing all the hours you have dedicated to the Health Scholar program. It will provide you with a complete breakdown of all your hours based on departments, meetings, and training. If you had taken on extra projects, then those hours will be included as well!

To start your audit process, you will have to complete the Hours Audit Request on CORE ELMS and email your designated Admin Coordinator to notify them of your request.

This audit of your total hours will take up to two weeks to complete to ensure accuracy and go through a final verification process. To complete your audit, our team will have to comb through all your data from each department you have shifted on and total up each individual hour to ensure it has been verified by the Departments branch. We do not want you to lose hours you have gained, so bear with us as we meticulously work through your hours!


Leave of Absence

Need to take a break from shifting for a serious reason? Inform us!

The Leave of Absence system is in place if there is a special circumstance that prevents you from shifting on the unit for a prolonged period of time.

To request an LOA, you will submit an LOA Request form on CORE ELMS and contact your assigned Admin Coordinator.

LOAs will NOT be approved for reasons pertaining to academia.

Program Completion Request (Graduation)

The Program completion request should only be completed if you are confident you will reach the total 280 hours by the end of the rotation. In the second month of each rotation, we will be reminding and having the deadline for the PCR to be completed if you wish to graduate from the program.

The PCR is also found on CORE ELMS under your Electronic Form, but to officially start the process, you must first have completed an Hours Audit Request. If you have completed and received the hours audit, you can then proceed to submit the PCR. Once you have completed the PCR, please also email sjoadaa@copehealthscholars.org to officially start your graduation process.

Letter of Recommendation

Let's be honest, most of us are here to get this!

The Letter of Recommendation (LOR) is a formal letter written by our St. Joseph Program Manager based on your performance with the program! Our Program Manager will compile all your achievements and performance with the program to draft up this letter. The exemplary awards and honorable mentions are taken into consideration for the letter. It pays to be exemplary!

To further enhance the quality of your Letter of Recommendation, consider joining the Leadership Team! Through being a leader, you will be able to showcase your talents and work ethics to our Manager, which only gives him better things to write about you!

To apply for the Letter of Recommendation, scholars must first have at least 240 hours with the program. At 240 hours, scholars may request the letter, but the letter will only be released to them once they complete 280 hours. As such, scholars may start requesting the letter when they start approaching their last few months with the program and are on track to graduate.

The admin team and Program Manager will work on the request and follow up to scholars regarding next steps.

Withdrawal Request

If you are unable to continue with the program and complete the 280 hours requirement, an option is to go through the withdrawal process.

Although not recommended, the withdrawal procedure can be completed if you are unable to continue with the Health Scholar program. Through withdrawing, you will not be eligible to receive the certificate deeming that you have completed the program with COPE Health Solutions.

To withdraw, the form on CORE ELMS will need to be completed and accompanied by an email to sjoadaa@copehealthscholars.org. Our Alumni coordinator will reach out regarding next steps.
