The Care Navigation Program offers two unique shifting experiences for our Care Navigators. Learn more below!

Shifts Available:

Monday - Friday: 8:30am-12:30pm & 12:15pm-4:15pm

  • 2 scholars per shift

Saturday - Sunday: 8:30am-12:30pm & 1:00pm-5:00pm

  • 1 scholar per shift

Case Management

Students will have the opportunity to:

    • Assist Patient Care Coordinators (PCCs) with the delivery of Important Message (IM) Letters for Medicare eligible patients within 48 hours of their anticipated discharge from St. Joseph

    • Access patient medical records to create Durable Medical Equipment (DME) packets in addition to delivery of DME to patients prior to their discharge

    • Schedule follow-up appointments for patients with their Primary Care Physicians (PCPs), specialists, or the Transitional Medical Clinic (TMC) following their discharge

    • Arrange transport for patients (i.e. Care Ambulance Services)

    • Learn more about the discharge process

Quality Excellence

Students will have the opportunity to:

    • Develop sought-after skills in Quality, Safety, Epidemiology, Risk, Analytics, and Patient Experience

    • Work hands-on with senior leaders in healthcare

    • Lead healthcare improvement projects

    • Earn the "Speak Up for Quality and Safety" certification endorsed by Providence St. Joseph Hospital at the end of the program

Shifting Information:

Shift times & days vary due to a number of factors, including students' availability and project.
