The Care Navigator program has led me to grow as a health scholar by opening my eyes to the administrative side of healthcare. This opportunity gives me the privilege of working alongside the hospital case managers and patient care coordinators to assist directly with their workflow to aid underserved populations. I have also transformed into a well-rounded individual by learning about how the Case Management Team works directly with nurses and physicians to improve patient care. As an aspiring physician, I found this program to be vital in my journey because it has allowed me to refine my interpersonal and organizational skills in order to be a valuable asset to the team. 

One of the most exciting and enticing things for me about the Health Scholar Program is the opportunity to develop myself and grow with the unique experiences that are offered. With the Care Navigation program, I feel that I honed in some past skills, but was also given a whole new skill set that I could bring with me to my career goal. Personally, I want to become a Physician Assistant, and although not everything I learned with Case Management can be directly translated to what is needed to become a PA, I still feel like everything I learned will be useful to me as a provider. It is easy to know that we need medical experience and knowledge to become a medical provider, but an unknown fact is that insurance, social determinants of health, and many other factors affect how a patient can heal and recover. Through my time with Case Management, I have learned many behind-the-scene knowledge that is not often seen from the perspective of a nurse, volunteer or even general Health Scholars. Each and every patient is unique and has individualized plans when it comes to discharging and their recovery process. From my experience with Care Navigation, I have seen the entire patient care team work together to collaboratively ensure what is best for each patient in the hospital, and this team heavily includes Case Management. Without this program, I feel that I would still eventually be exposed to this as a PA, but not to the level of detail that I know now, and not as early on as I have now. I can confidently walk away from this program saying that I have learned an incredible amount of knowledge that I would have never come across otherwise if I had not chosen to challenge myself for this role. These experiences will follow me very well into being a great future medical provider and was most definitely an experience I am glad I had a chance to partake in.

I learned a lot about the management of a patient’s care after their stay in the hospital through my time with the Case Management Team at St. Joseph. As a Care Navigator, you will be exposed to a different side of healthcare that is not talked about often. You will be tasked with setting up follow-up care for a patient, whether it’d be post-hospital appointments, Skilled Nursing Facility (SNF) placement, or delivery of Durable Medical Equipment (DME) to their home. While shifting as a Care Navigator, I had the honor of getting to shift on the Behavioral Health Unit. This is a new floor that was made available to Care Navigator Scholars and offered a new learning opportunity. On the Behavior Health unit, you will be able to not only assist the nursing staff with administrative tasks, such as making admission packets, but also learn how patients with behavioral health needs are managed differently than those without. As an aspiring health care provider, I find it interesting to know more about the peers that I will be working alongside and how I may provide the best care for my patients by utilizing all aspects of their care management. I highly suggest trying out Care Navigator if you are interested in learning more about the continuation of care after a patient visits the hospital and how social workers fit into that mold.
