Real Connections 

Description

As a school-based family resource center, Real Connections engages students and families to enhance campus climate and culture by developing student leaders and strengthening social/emotional confidence of teens, preparing them for college or post high school success.

Services

Elevate YOUTH (Open to public).  Groups for youth that are at risk for substance abuse disorders.  Refer to link for more information:  https://sites.google.com/view/saysdprogramguide/community-engagement/youth-support-groups/elevate-youth

Student attendance and behavioral support (Limited to Lincoln High School).  Assists school with students that are having attendence issues.  

• General Academic Support (Limited to Lincoln High School).  Tutoring and other services to support student in studies.

• Peer support groups (Limited to Lincoln High School).  Groups cover a variety of different social/emotional topics such as leadership, mental health, and other important topics important to our youth.

Case Management (Open to public).  Conducted virtually over phone or Zoom.  This includes basic needs support and referrals.  

One-on-One Mentoring (Open to Lincoln School Cluster school and Farb Middle School).  Utilizes COPE to Thrive, an evidence based curriculum.  Helps youth develop goals based on needs (academic, social, emotional, and/or other goals).  Allows a space for youth to process things with a trusted adult, organize aspects of their life, and learn to advocate for themselves. 

What I Wish My Parents Knew [WIWMPK] (Open to public).  A forum for youth to advocate for the needs of youth today and for parents to listen.  Refer to link for more information on this program:  https://sites.google.com/view/saysdprogramguide/community-engagement/youth-advocacy-groups/what-i-wish-my-parents-knew-wiwmpk

Eligibility

Mostly for students and families of Lincoln High School, the Lincoln Cluster, and Farb Middle School.  However, there are a few services open to the public.  Please refer to services breakdown above.

Service Areas

Located on the Lincoln High School campus, serving the Lincoln Cluster of schools and Farb Middle School.  Open for in-person by appointment, Monday-Friday from 8AM-4:30 PM.  Please in advance call to make appointment 

Elementary:  Baker, Balboa, Chavez, Chollas Mead, Encanto, Horton, Johnson, Nye, Porter, Valencia Park, and Webster

Middle School:  Knox and Farb

High School:  Lincoln and Millennial Tech
