CCC (Crawford Community Connection)

Description

Families that have students attending Crawford Cluster schools or live in the City Heights (92115 and 92105) community are provided referrals (housing, food sources, counseling, substance abuse, legal assistance, and more) and other services.

Services

• First 5 First Steps:  Point of contact is Iane Costa or Liliana Ruiz at 619-362-3756. 

• HHSA MediCal, Cal Fresh, CalWorks enrollment:  Point of contact is Deirdre Duenas at 619-362-3755.  She is only accepting calls on Wednesdays from 8am-4pm.  If an individual wants to know the status of their MediCal, Cal Fresh, Cal Works application, they can call, make an appointment, or walk in on Wednesdays.  For hours outside of Wednesdays, please have the individual call the Access Line at 1-866-262-9881.  The HHSA Representative can do the following:

• Assist customers with the paperwork for SAR reports and renewals (But cannot process them)

• Assist with AND process applications for Calfresh, Medi-Cal and Calworks.

• Check application status and advise if anything is needed.

• Resource connections (Healthy Start):  Case management for families connecting and linking to services.  Point of contact Iliana Salas at 619-362-3752. 

Eligibility

Serving all students and families in City Heights (92115 and 92105), particularly those attending Crawford Cluster schools.

City Heights (92115 and 92105) and Crawford Cluster schools.  Currently open for in person by appointment Monday-Friday 8:30-4:30

Crawford Cluster Schools are:  

Elementary Schools:  Carver, Clay, Euclid, Fay, Ibarra, Marshall, Oak Park, and Rolando

Middle School:  Mann Middle School

High School:  Crawford High School

Health Community Services Representative can service ALL zip codes throughout the county with Medi-Cal/SNAP/EBT questions and application.  

We suggest to clients when calling if, the Health Community Services Represenative does not answer, be sure to share the following when leaving a message:

• What program they are calling about (ie. CalFresh, MediCal)

• Name

• Phone number

• Date of Birth
