What you are going to need:
Meting minutes stating the following:
Date of the meeting
The Organization name (All checks should be made payable to this name)
Purpose of the Organization
All Officers of the organization and their title
Authorized signed for the account and tile
Faculty advisor's name and signature
Signing Officer's names and signatures
Tax identification number
UNLV Taekwondo should already have a number this can be found following the following for or contacting Student Involvement and Activities

If needed a new one you can request one at IRS EIN Web Site
All accounts signers MUST be present at the time of account opening with personal identification including:
Social security number
Present address
Phone number
Email address
Proper identification for each signer (1 for each signer):
State ID
US Military ID
Foreign passport
Canadian driver license
State driver license
US Alien registration – immigration passport
Mexican consulate/Matricula ID
$100 minimum opening deposit

Tax information, including guidance on filling out forms like the W-9, can be found on the UNLV website. Please utilize these resources to ensure accurate completion of tax forms and compliance with relevant regulations.
UNLV Taekwondo should already have an EIN. If this is lost, you can contact SIA to get a copy. There should also be a copy of this in the Google Drive. In case it cannot be recovered or you need to obtain a new one for any reason, you can apply for a new EIN on the IRS EIN website.

In order to transfer the EIN number, you are going to have to fill out a form through the IRS. For instructions on this form, you can refer to IRS update contact information.

Announcements (and/or purpose of the meeting)
List all the announcements made at the meeting. For example, the purpose of today's meeting is to discuss the opening of bank at the U.S. Bank, the documentation required for a Non-profit Organization account, and the signers authorized to be an the account. (If you are charging signers on an existing account list the names of the signers to be added , who will remain on the account, and who is to be removed.)
Discussion
Summarize the discussion for each issue, state the outcome, and assign any action items.
Name, titles and signatures of all signers for the account.
Faculty advisor name, titles and signatures.
