Reporting Culture

Overview

A reporting culture is one where club members have confidence to report safety concerns or gliderport hazards without fear of reprisal. Reports are acted upon whether they are submitted anonymously or not. SCOH as has an online safety reporting system that is available to members.

SCOH is not able to fully understand operational hazards without direct input from members working and flying during day to day operations. We are committed to cultivating an atmosphere where all members have confidence in reporting safety concerns without fear of reprisal.

Our goal is to create a robust reporting culture that becomes established through continuous use. All SCOH members are encouraged to continuously utilize available voluntary member reporting programs.

Learn from Ourselves

Reporting is actively encouraged with an emphasis on learning from these reports rather than blaming those involved.

As the SCOH safety culture matures, we must understand that human error is viewed as a symptom, not a cause of incidents. Subsequently, we must view the full context of human factors within systems and processes to better understand all variables contributing to an incident.
