River City ARCS members who are current may request a membership badge.

You may fill and submit the online form below on this web page. 
Be sure to scroll to the bottom and click the "Submit" button. 

Otherwise, you may print this PDF Form or download and print this MS Word form and mail or deliver it at our regular meeting. 

Your laminated name badge is generally prepared within one week of its request and ready for you to pick up at the sign-in table at our next in-person activity or membership meeting.
