Why are they always missing?

Trolls.

Replacements are available at the dealer(can be the cheapest option with this part), eBay, JY(almost never find them at the JY), etc, look for part number 10374699 and 10374700 GM calls them retainers or something like that.

Astro Pop over on AstroSafari.com used to make a custom acrylic cupholder for the 96 and up vans, he has unfortunately gone out of business.

Some have used the rubber Fernco fittings(used to join pipe) 3" no-hub coupling, pvc, plexiglass, or even empty duct tape rolls to enhance the vans ability to retain a cup.

These used to be available, it would be pretty easy to copy the design and make your own.
