SHAPE Questionnaire

Review the supportive information on the Apprentice Space Website and then complete the SHAPE questionnaire. This questionnaire is very short and only takes a couple of minutes to do, but it will ensure you have sufficiently read about the SHAPE themes by questioning your knowledge about the things you have learned on this website. When you have completed the questionnaire, click on View Score and look at the Total points you have achieved.

Step 1: Complete the quiz and check your score

The pass rate is 5/6 points, so if you pass at the end of the questionnaire, DO NOT CLOSE the questionnaire, keep it open so you can follow the steps below to preserve a copy of your score sheet to be uploaded to Smart Assessor.

Step 2: Check your score

When you have completed the questionnaire, click View score to see if you passed.

If you notice your score is at least 5/6 points, proceed with the next step.

Step 3: Print your results

If you passed the questionnaire, save your results. To do this, click CTRL P on your keyboard to open the Print menu (Apple Macs may have a different key combination).

In the Print menu, change your selected printer to one of the following:

Print to PDF
Microsoft Print to PDF
Save as PDF
Adobe PDF
Any printer option that appears to offer saving/printing to PDF
If you don't have any options for this, try Microsoft XPS Document Writer if avaialble

When you click Print, you will then be asked to save the PDF to your computer/device.

Step 4: Upload your questionnaire PDF to Smart Assessor

In Smart Assessor, select the SHAPE course and go to Upload Work. Upload your Questionnaire printout, and map it to the SHAPE course against the Questionnaire criterion.

Technical problems

If for some reason you are unable to print your results, perhaps because your device does not support PDF printing, contact smartassessor@sheffcol.ac.uk so the administrator can access your results for you.
