URISE Temporary or Permanent Withdrawal Form
Communication is key to the relationship between Trainees and URISE. If Scholars need to withdraw or consider their options they can complete the withdrawal form, reach out to Carlos Quatela, or reach out to another URISE Staff/Faculty member.
The EXITO/URISE Withdrawal Form collects information needed for temporary and permanent withdrawals. URISE requires submission of the form each time a Trainee withdraws.
Some information on the form is required for URISE to meet NIH reporting requirements and other information will help URISE provide support during your withdrawal. Carlos can complete the form for you in a meeting.
Temporary Withdrawal (aka Leave of Absence)
Definition
The Trainee decides to be inactive in most or all program requirements for one or more terms.
Changes to Program Requirements/Benefits
Participation in program activities is not required for the withdrawn term(s)
URISE financial support is forfeited for the withdrawn term(s)
Trainees still have access to the URISE community including advising and the STEM Center.
If you temporarily withdraw, URISE can not guarantee a spot in the research training program or future URISE funding.
Process to Request a Temporary Withdrawal (AKA Leave of Absence)
If a Trainee is dealing with an exceptional amount of personal challenges, taking a leave of absence from the program may be the best option for the trainee. Requests for temporary withdrawals will be approved by URISE leadership after all other options have been explored first. To request a temporary withdrawal:
Meet with Carlos Quatela to discuss agreements between the program and the Trainee while the Trainee is on leave.
If URISE Leadership approves the temporary withdrawal, URISE will draft two documents:
A leave of absence request to NIH, which must be signed by the Trainee and URISE's PI, and
A letter outlining the agreements made between the program and the Trainee for the duration of the leave of absence
URISE amends your Trainee Appointment with a new end date. This must be routed to the PI, and the PI routes the Trainee Appointment to NIH.
Requirements While Temporarily Withdrawn
URISE is dedicated to allowing for Trainees to disengage with URISE during temporary withdrawal if that is what they need. And, Trainees must keep lines of communication open with the program during their temporary withdrawal. There are a lot factors (specific to your circumstances) that need to be considered when withdrawing temporarily.
Trainees are required to do the following during temporary withdrawal:
Quick check in with URISE (email or short meeting with URISE Advisor) to do the following:
Update URISE on their plan to return to the program, including a timeline
Let URISE know if they need any support
URISE will let them know if there are any requirements for their term of withdrawal and any information about returning to the program
This meeting must happen in the first 4 weeks of each term
Notify Carlos Quatela of your return in a timely manner
Trainees are required to notify URISE six weeks before the return term to allow for the OVV badging process.
Review the offboarding email URISE sends after notification of temporary withdrawal.
Resident Tuition & Temporary Withdrawal
Non-resident transfer Trainees from BUILD institutions (e.g., Clark, NMC, ASCC) will be assessed RESIDENT tuition rate even if they are non-residents. Trainees will have this benefit as long as they are active in the program and for two years after program completion (undergrad only).
If a Trainee is planning to withdraw for one term only, they will retain the URISE resident tuition rate for the withdrawn term.
When this occurs, the Trainee will not have access to the one term of resident tuition post program completion.
If a Trainee extends their withdrawal past one term, they will lose resident tuition until they return to the program.
If the Scholar is planning on a temporary withdrawal of 2+ terms, they will lose resident tuition until they return to the program.
Permanent Withdrawal
Definition
The Trainee decides to withdraw from all program activities.
Changes to Program Requirements/Benefits
Trainee ceases to be part of their URISE Cohort, and participation in all program activities ends on the date agreed by the Trainee and the program.
URISE financial support is forfeited.
Access to the STEM Center is revoked.
Resident Tuition and Permanent Withdrawal
Non-resident transfer Trainees from BUILD institutions (ASCC, Clackamas, Clark, NMC, PCC) will be assessed RESIDENT tuition rate even if they are non-residents. Trainees will have this benefit as long as they are active in the program and for two years after program completion (undergrad only).
If a Trainee eligible for URISE’s resident tuition withdraws permanently they will lose access to URISE resident tuition. Their tuition rate will be changed once the change in status email is sent to Financial Aid.
