Trainee Documents
Before you begin your first year as a URISE Trainee, you must complete onboarding paperwork.
There are several types of forms and digital paperwork to complete to be established as a URISE Trainee in PSU and NIH systems. Each of the items below are necessary to be fully onboarded as a URISE Trainee.

Confirming Banweb information
In order to set up your monthly stipends, we need to ensure your personal information in BanWeb is accurate.
CIMR FERPA Consent & Work Product Release Form
For compliance purposes and to complete administrative forms necessary to establish your research training, we sometimes have to re-disclose identifiable information. The FERPA & Work Products Consent Form records your consent to:
To re-disclose your personally identifiable program information and your student record with NIH and NIGMS; and
To reproduce or share your research products and likeness captured during presentations outside of the CIMR community for marketing purposes
Fall Enrollment
Due to NIH policy, you are required to be registered full time in Fall 2023 to participate in URISE summer activities. Please make sure you are registered full time for Fall 2023 by June 15. If you have any questions or need assistance registering, please contact Carlos Quatela (scheduling link)
Stipend Form
The Stipend Form allows URISE to process your monthly stipends.
Forms will come from adobedesign@adobedesign.com on or around Friday, June 2.
Subject Line: “PSSP 2005CC Last Name First Name $1195”
Forms are signed electronically via Adobe
Forms must be signed by Thursday, June 15 at 12:00pm
Review all information on the form to ensure accurate information:

Total Payment to Participant: $14,340
Total Number of Payments: 12
Payment Amount: $1,195
If information is incorrect or you have questions, please emails urise@pdx.edu and spa_cimr@pdx.edu as soon as possible.
Stipend forms must be signed and returned no later than 12:00pm on Thursday, June 15.
Please note that monthly stipends will not begin until this form is signed and returned electronically.

The next step in onboarding is to create your NIH Trainee Appointment. In order to create your Trainee Appointment, the following forms/tasks must be complete:

Summer 2023 Trainee Agreement
The Trainee Agreement outlines the responsibilities of being a URISE Trainee.
The Trainee Agreement is our way of entering into an agreement with you about the program requirements and expectations.
Please do the following:

Once all your questions are answered, please follow the instructions on the form to sign the Scholar Agreement.
