Educator Crowdfunding
CCS educators have the option to use crowdfunding platforms such as DonorsChoose to raise funds for educational expenses.
If you choose to crowdfund then be sure to go to CCSDAS and submit a request under "Crowdfunding" to document your crowdfunding campaign.
Be sure to review the guidelines and trainings that are linked in the request form.

FCCS has created an online platform for mandated reporters (such as CCS), to allow online reporting of abuse or neglect.

Home Instruction
Home Instruction
Below is some helpful information regarding Home Instruction. Home Instruction is typically indicated for students who are expected to be out of school for at least 20 consecutive school days due to medical reasons. Here is the general process:
Home Instruction application (2 pages): This includes an Authorization for Release of Information form and a Medical Application for Other Health Impairment Eligibility or Home Tutoring. (See links below)
Authorization for Release of Information: Requires a parent/guardian’s signature.
Medical Application: Should be completed by the medical provider.
Submission: Once completed, the paperwork should be forwarded to the Columbus City Schools Home Instruction Office at homeinstruction@columbus.k12.oh.us for review (either you or the doctor’s office may email it)
After review, a Home Instruction representative will contact the family regarding the next steps.
