Explanation
In between a student's annual IEP team meetings, the IEP team may decide to make changes to parts of an IEP rather than rewriting the entire document. This is amending the IEP. This may result from a team meeting, but also can be done without one as long as communication with the parent has occurred. An IEP team meeting is not required for all amendments (more info. on this in the FAQ section below).
Completing an IEP amendment
When amending an IEP, you will send 2 documents home to the parent:
Amended IEP
Prior Written Notice (PWN) describing the amendment
***NOTE: To amend an IEP in SpEd Forms, the student needs to have a complete finalized IEP in SpEd Forms. If your student in need of an amendment does not have this yet, contact your building Supervisor/Manager. Keep in mind that a partial IEP for data entry is not complete enough to amend as is.
Check the "Amendment" box on the IEP menu.
On the top of the Student Information page (page 1 of the IEP), fill in the amendment date.
Make the additional changes to the IEP that you are making as part of the amendment. (Exmaples: services & LRE, testing accoommodations, etc.)
Validate the IEP.
You will finalize the amended IEP after you receive consent or 14 days has passed.
On the forms menu, choose "Prior Written Notice/Consent Part B."
Complete the PWN. There are drop down statements for amendments to help you. (Reminder: Drop downs are beginning statements that must be individualized - describe what was amended!)
Finalize the PWN after you receive consent or 14 days passes.

No. Put an end date on the old services (date should be the date in which consent occurs and the amended IEP is in place). Add the new services. The start date should be the date in which consent occurred and the amended IEP is in place. The end date of the new services should be the OLD end date from the OLD services - in theory, this was the end date of services on the last annual IEP.
When an amendment is needed, the case manager must ask the parent/guardian if they would like an IEP team meeting. If so, complete NOTM and hold the meeting to discuss the amendments. If the parent/guardian does not want a meeting and is comfortable to discuss changes with the case manager or service provider, include this in the PWN (Explanation: District is proposing an amended IEP. Parent/guardian declined a team meeting and amendments were discussed during a phone conversation with the parent/guardian on XX/XX/XX ). Document the phone call in the communication log in SpEd Forms.
See question above! The parent/guardian has the right to call an IEP meeting anytime. It is the case manager's/service providers responsibility to offer a meeting when inquiring about an amendment.
This is really up to the team, but in general, if an IEP requires many changes to goals, services, and LRE, it is best to hold an annual meeting and write a new IEP instead of amending an old IEP. The new IEP meeting date will become the new annual date.
