Explanation
The Student Information page is page 1 of the IEP in SpEd Forms. It is important that this page be accurate as it contains key information that is reported to the state and is used for planning and programming. Many fields on this page are auto-filled in from information imported into SpEd Forms from our Synergy system. This includes the student and parent information. Other fields need to be completed by the case manager when writing a new IEP.
Fields to Complete
Dates: The last IEP date is for last year's annual IEP date. If this is an initial, this field would be blank. The date of last comprehensive evaluation should autofill with the date of the finalized evaluation or the date entered by leadership on the set up page. Check for accuracy. The IEP meeting date is for the current/most recent IEP meeting date.
Note: A common error has been having the wrong date in the last IEP and/or IEP meeting date field. Check these before finalizing!
Under the Student Information header, check information and update as needed for accuracy.
If there is incorrect information under the Parent/Guardian Information header, contact Krystal Jenson to have Synergy & SpEd Forms updated.
Under the IEP Information header, complete all fields accurately. Be sure that the disability and instructional setting field are correct based on this new IEP. If there is a change in either area, report it to Krystal Jenson as it is a MARSS change. You only need to use the date and second instructional setting fields when there will be a second setting during the life of this IEP. Otherwise those should be blank. Additional setting guidance is below.
Under the Progress Reporting header, choose the appropriate drop down (s).
Under the IEP Team header, ensure the list accurately represents those that were present for the meeting. This requires you to edit the list shown. Reminder, you can find SpEd and Admin. staff by clicking the person+ icon.

BCCS K-12 Federal Setting Cheat Sheet
This cheat sheet will help you determine the federal setting for a student.
First determine services.
Then calculate how many minutes per day or week the student will receive DIRECT services in the SPED ROOM setting.
Then reference the chart for the ranges for each setting.
Tip: You do need to calculate setting on your own, but you can click Instructional Setting in SpEd Forms to see the list and click the appropriate number.

This information imports each night from Synergy into SpEd Forms. Therefore, letting the building person know will lead to SpEd Forms being updated as well. If there is a general address or phone number change, contact the building staff listed below.
