Explanation
This page on the student's IEP includes accommodations, modifications and supports needed in both the general and special education setting. The page also includes any supplementary aids or services needed in non-academic or extra-curricular activities and program supports for school personnel. Although there are drop downs for staff to use in SpEd Forms on this page, the items entered must be specific to the student, their disability and their individual needs. Service providers are responsible for entering their own information on this page if it is specific to their service or area of support.

What are program supports for school personnel?
In order to complete this section, during the IEP meeting, teams should consider:
What professional learning (e.g., training, coaching, conference attendance) do school personnel need to execute the IEP, promote progress in the general education curriculum, and support the student’s development of peer relationships?
What special equipment or materials are necessary for personnel to implement the IEP, support student progress in the general education curriculum, and promote student access to peers?
Is planning time with other staff or administrators necessary to implement the IEP?
Does the classroom teacher need additional assistance in the classroom (e.g., having an aide in the classroom) to effectively support the student’s access to and progress in general education?
