When An Employee Requests a Job Accommodation
When an employee requests an accommodation to her/his job that falls into one of the following categories:
A long-term medical condition or a disability, including pregnancy, childbirth and pregnancy/childbirth related medical conditions.
Bringing a service animal to work.
Please complete the Job Accommodation Notification Memo form below and submit it immediately to provide the employee with a memo containing legally compliant instructions and then follow up with her to ensure she acknowledges receipt of the memo within 24 business hours.

Request for Medical Accommodation Form: Form that employees use to request a job accommodation due to a medical condition. Please print this form for employees who cannot access the HR Site for Employees. (Three separate forms are embedded within this one.)
When You Learn an Employee is Pregnant
When you learn an employee is pregnant, please complete the Pregnancy Notification Memo form below and submit it immediately to provide the employee with a legally-required memo of notice and then follow up with her to ensure she acknowledges receipt of the memo within 24 business hours.
When an Employee Returns from Parental Leave
When an employee returns from Parental Leave, please notify payroll@nkyhealth.org and hr@nkyhealth.org immediately.
