NKY Health uses AdobeSign to route and track documents that need to be signed. Anyone who has an account can set up a document for signature.
Managers/Supervisors can also use custom designed workflows to facilitate some routine NKY Health workflows.
Please contact the head of Human Resources to set up an account.
General Workflow Steps:
Log into AdobeSign
From the main screen click "Start from Library".
From the menu on the left click "Workflows."
Select a workflow.
Click "Start".
Follow the instructions on the wizard.
Add the individual's last name to the subject line of the message. (This makes tracking easier.)
Select "Sign then Send" to make the form fillable.
Complete all the required fields. (NOTE: The data on the form SHOULD save if you exit and come back but it may take a few minutes to show up.)
Click "Send."
Available Workflows:
Position Allocation Form: Please initiate this form when you want to fill a vacant position. Be sure to attach the Job Description for the position to the form.
Background Check Authorization: Please initiate this form for any volunteer you want to bring on board at NKY Health. Send to the volunteer at her/his e-mail address.
NKY Health Application for At-Will Employment: Please initiate this form after you schedule an interview with an applicant. The form must be completed and signed at least 24 "business" hours prior to the interview. Send to the applicant at her/his e-mail address.
Performance Evaluation Tool: Please use this form as the Performance Evaluation Tool to conduct Introductory and Annual performance evaluations. Complete all the information before sending for signature from the Division Director and the District Director of Health, then hold and forward to the employee for her/his signature after you meet with her/him.
Performance Evaluation Delay: Please use this form sparingly to document in advance when a Peroformance Evaluation will be delayed due to unavoidable circumstances.
Workers Compensation: Please initiate this form when an NKY Health employee becomes injured or ill as a result of something that happened while working at NKY Health.
AdobeSign may allow you to complete a workflow in multiple sessions (but it's not always reliable in this respect.) If you need to stop and restart, try following these steps:
Login.
Select X Waiting For You (X = a number) to see the list of workflows in progress.
Select Open Item to open the form. (Note: It may appear to be blank.)
Select View and Sign to open the form where you left off.
Always start from a workflow. This is the most important tip! Instead of editing a template, start your form from a workflow. Editing a template directly will change it for everyone at NKY Health--yikes!
Give your documents a clear name. Add a specific name to the subject line of every form, document, or workflow you start. This makes it much easier to track what you're working on.
Use "Sign then Send" to make edits. When you need to edit fields on a form, select "Sign then Send." This opens the form fields for you to make changes before it goes out.
Try to complete your form in one session. AdobeSign doesn't always save your progress correctly if you log out in the middle of a session. It's supposed to save, but it can be a bit unreliable, so it's best to finish in one sitting.
Be aware of scrolling text. Text that scrolls within a field will not appear on the final printed version. If you have a lot to write, type "See attached" in the field and attach a separate file. You can even use specific section titles in your attachment (e.g., "See Section XYZ of attached").
Combine your files. Many forms only allow one file attachment. If you need to include multiple documents, try combining them into a single PDF or word file before you attach them.
You can't edit a sent form. Once you complete and send a document for a signature, you can't edit it. However, you can cancel it and start over. You can even download the old document and copy-paste the information to the new one.
You can reroute a document. If you initiated a form, you have the ability to reroute it to an alternate person for signature if needed.
